Preparing for Acute Care Situations

 
The downloadable ‘Marie’ in Hospital Profile illustrates the profile of a patient moving from a domiciliary setting into hospital. Such profiles can help a patient or their family/carers, worried about how well they will cope in the future, prepare for possible future care interventions.

Such material can enhance hospital or residential/nursing care so that:

  • Patients that can communicate effectively are cared for in a dignified and respectful way
  • Patients with confusion, memory loss or dementia are cared for as unique people.

A person, recovering from a serious surgical or medical problem may wish, in the comfort of their own home, to communicate to an advocate their personal wishes about what is important to them in their daily lives.

Their family/friends may also add their experience in helping them cope with daily living situations.

These communication and an observation profiles can be lodged with their GP records for use should they at some point be admitted to hospital or to residential/nursing care.

It is also possible for acute hospitals to enhance their patient admissions, so that on admission hospital resources are committed to collecting and combining key aspects of patients communication and observation profiles for use on the wards - and staff are trained to use them.
